With the increasing problem of AIDS, a wide variety of radiological features are being seen. The early findings in the process of this disease are sometimes very subtle, the patient may not even know that he is infected with the virus. The most common finding is simply cerebral atrophy. The most important complications are related to opportunistic infections as well malignancy. The most common opportunistic neuroinfections are toxoplasmosis, criptococcosis, tuberculosis, cytomegalovirus and the fungal agents. MRI is highly sensitive in their detection, specially when used with gadolinium infusion. The knowledge of the great variety of neuroimaging features will enable the radiologist to suggest this entity even in those cases clinically unsuspected.
