The occurrence of high frequency oscillations (HFOs) between 80 and 500 Hz is closely linked with epileptogenic areas. HFOs were originally discovered with intracranial micro-electrodes, but have, in the meantime, been identified in standard clinical intracranial electrodes and on surface EEGs. Several studies have shown a correlation between the removal of HFO generating tissue and postsurgical outcome. This review focuses on HFOs and their value as a biomarker of epilepto-genicity. Various clinical applications of HFOs and methodological problems are discussed. In conclusion, HFOs are promising EEG markers in epilepsy. Multicenter prospective studies are, however, necessary to evaluate clinical relevance and reliability of HFOs prior to widespread clinical use.
