Arthroscopic anterior cruciate ligament reconstruction with the patellar tendon for chronic laxity: follow-up of 69 cases

Assessment with the IKDC form of 69 cases of arthroscopic anterior cruciate ligament reconstruction with the patellar tendon for chronic laxity after a 2-yr minimum follow-up showed 80% excellent plus good results, together with subjective and objective satisfaction in 88% and about 91% respectively. Pain was absent in 87%. Effusion accompanied by quadriceps hypotrophy or a meniscus disorder was observed in 10%. Resumption of physical activity or sport at the same level was reported by 78% of the patients. Giving way was described by about 6%. The Lachman test was positive in about 6% and the pivot-shift abolished in 97%. The incidence of meniscus lesions was: medial 30%; lateral 10% both 6%. Cybex II evaluation showed that torque and muscle strength were equivalent in both limbs in about 75% of patients, whereas problems stemming from failure to follow the rehabilitation programme and/or concomitant meniscus disorders were noted in the other 25%. Radiographic examination indicated incorrect placement of the tibial hole in 6%, with impingement in 4.5% coupled with 5 degrees-15 degrees loss of extension, but no loss of flexion.
