Infective endocarditis (IE) is the fourth leading cause of life-threatening infectious disease. One of its most important neurologic complications is the symptomatic intracranial mycotic aneurysm (ICMA). Patients with endocarditis and intracerebral hemorrhage should be evaluated with angiography for a possible ICMA. The primary treatment is administration of antibiotics for 6 weeks. We report a patient with IE who, having presented with neurologic signs and symptoms, was found to have an ICMA. We summarize the recent literature on diagnosis and treatment of ICMAs.
