The clinical manifestations of Waldenstrom's macroglobulinemia are related to direct tumor infiltration and to the unique properties of the immunoglobulin M monoclonal protein. Plasmaphereses usually combined with systemic chemotherapy provide effective palliation for most patients. Standard treatment consists of oral chlorambucil, but the nucleoside analogues cladribine and fludarabine are very active in this disease. These agents are the treatment of choice in patients with disease that is resistant to alkylating agents.
