Transmastoid facial nerve decompression for recurrent facial palsy

Five patients suffering from recurrent facial nerve palsy (RFP) were treated with transmastoid surgical decompression of the facial nerve from the site of the lesion, as determined by means of topognostic tests, to the stylomastoid foramen. All subjects reported excellent recovery (greater than or equal to 80%) of the facial function and, followed up for 7 to 18 years, were free of recurrence. These results confirm the efficacy of facial nerve decompression for RFP; they seem to suggest that not all patients with RFP need a total decompression of the facial nerve, including meatal and labyrinthine segments, to recover facial function and prevent recurrence. Further experience is necessary to assess the choice of treatment in patients with RFP.
