Laparoscopic myomectomy and both uterine artery occlusion

Objective: The purpose of this study was to evaluate the results of laparoscopic myomectomy with both uterine artery occlusion in treating symptomatic myomas. Methods: From March 2006 to Feburuary 2007, twenty women with symptomatic myoma were treated by laparoscopic myomectomy. We infiltrated vasopressin on myomas for prevention of acute bleeding during operation. And for avoiding delayed bleeding, hematoma and recurrence, we occluded both uterine artery by using endoclip after myomectomy. Results: Average age of patients were 41 years, mean BMI was 22.7, and average number of myomas was 2 (1-4). The weight of myoma was 128.5gm (29 similar to 333gm). Mean operation time was 105 min (60 similar to 150min). And mean blood loss was 105ml (10 similar to 300ml),and mean difference of Hb level between preoperation and postoperation Hb was 1.6 gm/dl (0.1 similar to 3.7gm/dl). Mean postoperative hospital stay was 2.6 days (1 similar to 6days). Major intraoperative and postoperative complications were absent. Delayed bleeding, hematoma and recurrence were absent. And in all patients, symptoms were much improved. Conclusions: Laparoscopic myomectomy with intraoperative vasopressin infiltration and both uterine artery occlusion was considered as safe and effective methods for treating myoma.
