A case report of ovarian metastases from breast cancer appearing under the mask of primary ovarian cancer

Breast cancer is the most common malignant solid tumor among women (30% of all malignant diseases). Every year 114.9/100 000 women are diagnosed with it. This tumor metastases both hematogenic and lymphogenic. Through the vessels the primary cancer cells spread and colonize distant organs. One of the rarest metastatic sites is the ovaries. However, there is a number of similarities between ovarian metastases from breast cancer and primary ovarian cancer. Differentiating the former from the latter is difficult. We present the case of 57-year-old female with tumor formations in the ovaries, who 14 years ago was underwent mastectomy and chemotherapy due to breast cancer. This formations and ascites in the pelvis were detected with PET scan. Vaginal ultrasound confirmed bilateral enlarged ovaries 5x5cm and ascites in the Douglas pouch. Taking the patient's symptoms into consideration, there is a chance the tumors will be pri-mary ovarian cancer or ovarian metastases. Laparoscopic bilateral adnecsectomy was performed. The histological report shows metastasis from a ductal breast cancer in both adnexa. Ovarian metastases from breast cancer are a rare condition. Distinguishing them from the primary ovarian cancer is very difficult, but also very important due to the different ap-proach in both surgical and postoperative treatment. Surgical removal of the metastatic ovarian improves life expectancy only in premenopausal women, while in menopausal women it is with diagnostic purposes only.
