The objective of this article is to discuss the case presentation of a 50-yr-old man who suffered atraumatic osteolysis of the distal clavicle. The patient felt the pain in the lateral shoulder region, and it extended into the midportion of the upper trapezius. Movement aggravated the symptoms. Ultrasound, interferential current, and cryotherapy relieved the subacute symptoms of inflammation and pain. Modification of his exercise regimen allowed him to continue his activities, while reducing irritation to the acromioclavicular joint. Muscle energy and facilitation techniques, Active Release Techniques, and strengthening of the appropriate muscles all contributed to this patient's successful rehabilitation. The type of multifaceted conservative treatment approach used in this case is recommended for the significant number of patients, with atraumatic osteolysis of the distal clavicle who do not need or want to undergo anything as extreme or as costly as surgery to correct this condition.
