Chronic lower extremity ischemia is a significant source of morbidity and remains a consistent predictor of cardiovascular mortality in the general :population. Increasingly peripheral arterial disease is treated by endovascular means, however, open surgical revascularization remains a gold standard especially when patency is the primary end point especially in the infrainguinal region. We describe our approach at the Brigham and Women's Hospital, Harvard Medical School, Boston MA, USA in the evaluation, surgery and postoperative care of patients undergoing infrainguinal bypass.
