摘要
Heart transplanted person and family experience different kinds of transition, and it ' s essential empower them for a healthy transition. Rehabilitation Nursing (RN), due to its skills, can help people to regain stability and well-being, however there is little dissemination of its foundation in nursing theories. Objective: To analyze contribution of the Transitions Theory in the practice of rehabilitation nursing aimed at empowering heart transplanted person and family. Methods: Critical-reflective study guided by Transitions Theory by Afaf Meleis and regulatory legislation for rehabilitation nurse practice. Results: It's understanding that Transitions theory supports Rehabilitation Nursing intervention in areas as knowledge and skills learning of heart transplanted person and their family, leading to their empowerment, decision make and originating action, making person feel capable to. Conclusion: It ' s essential to demonstrate conceptualization in nursing practice being transition a significant focus for Rehabilitation Nursing intervention.
