The shiitake mushroom (SM) is native from East Asia. The presence of SM has gained an enormous popularity among western countries in recent years, turning adverse reactions to SM intake into a more common event. Despite most of the reactions reported after SM intake are mainly flagellar dermatitis, no IgE-mediated reactions have been described yet. We present the first case reported of proven IgEmediated urticariform reaction in a western patient after ingestion of shiitake mushroom. Given that increased consumption of this food is taking place in western countries, allergists should be aware of SM allergenic potential and thus it should be taken into account during the differential diagnosis of food allergies.
