Odontogenic fibromyxoma is a rare variant of odontogenic myxoma with more dispersed collagen fibers. This tumor shows different radiographic patterns, including "soap bubble," "tennis racket," and "honeycomb". The "sunray" or "sunburst" is an infrequent radiographic pattern. The rarity of odontogenic fibromyxoma and this radiographic presentation as a usual presence in malignancies such as osteosarcomas leads to confusion in the differential diagnosis of the lesion with a radiological presentation of "sunray" appearance before doing histo-pathological evaluations. Therefore, reports of such cases make clinicians facing unusual radiographic mani-festations include better diagnostic and treatment choices. This case report presents the diagnosis and surgical treatment of a mandibular central odontogenic fibromyxoma in a 17-year-old female who has a rare radiographic finding as a "sunray" appearance.
