Superiority of the Septal Extension Graft Technique in Rhinoplasty Patient Satisfaction Versus the Intercrural Strut

Background: Considering that rhinoplasty is one of the most demanded surgeries nowadays, it is necessary to analyse the relationship between the final result and the satisfaction of the patients who have undergone surgery. The objective of this study was to measure patient satisfaction by comparing the results achieved 6 months after their surgery following the use of two surgical techniques: the intercrural strut type technique, in contrast to the septal extension graft technique. Methods: Ambispective analytical cohort study in two parallel groups with patients scheduled for rhinoplasty. Sample size was calculated with a confidence level of 95% and a power of 80%, resulting in a minimum of 20 subjects to be included. Fifty-three patients participated: 22 operated with the intercrural strut technique and 31 with septal extension grafting. Results: An increase in the nasolabial angle was identified, with similar results between techniques. At 6-month evaluation, a decrease in the nasolabial angle was observed in all patients. Conclusion: The nasolabial angle remained higher at 6 months in the septal extension group compared to the intercrural post group; however, these differences were not statistically significant. Notably, the patient satisfaction survey revealed that only 59.1% of individuals who underwent surgery with the intercrural strut technique expressed satisfaction with the results. Conversely, 100% of the subjects who underwent surgery using the septal extension graft technique reported being satisfied with the final outcome. Contexte : Considerant que la rhinoplastie est l'une des interventions les plus demandees actuellement, il est necessaire d'analyser le rapport entre le resultat final et la satisfaction des patients ayant subi la chirurgie. L'objectif de cette etude etait la mesure de la satisfaction des patients en comparant les resultats obtenus 6 mois apres une chirurgie ayant utilise l'une des deux techniques chirurgicales suivantes : technique de type entretoise intercrurale ou technique de greffe d'extension du septum. Methodes : etude de cohorte analytique ambivalente de deux groupes paralleles avec des patients devant subir une rhinoplastie programmee. La taille de l'echantillon a ete calculee avec un intervalle de confiance de 95% et une puissance de 80%, montrant la necessite d'inclure au moins 20 patients. Cinquante-trois patients ont participe; 22 ont ete operes selon la technique d'entretoise intercrurale et 31 avec greffon d'extension septale. Resultats : Une augmentation de l'angle nasogenien etait identifiee et les resultats des deux techniques ont ete comparables. Lors de l'evaluation a 6 mois, une diminution de l'angle nasogenien a ete observee chez tous les patients. Conclusion : L'angle nasogenien est reste plus important a 6 mois dans le groupe d'extension septale comparativement au groupe de support intercrural; toutefois, ces differences n'ont pas ete statistiquement significatives. Il est interessant de noter que l'enquete de satisfaction des patients a revele que seulement 59,1% des patients ayant subi la chirurgie par technique de l'entretoise intercrurale ont manifeste leur satisfaction envers les resultats. En revanche, 100% des patients ayant subi une greffe d'extension septale se sont declares satisfaits du resultat final.
