股骨颈动力交叉钉系统与空心螺钉治疗中青年股骨颈骨折的疗效比较

摘要
目的比较股骨颈动力交叉钉系统(FNS)与空心螺钉治疗中青年股骨颈骨折的临床疗效。方法采用回顾性队列研究分析2018年3月至2020年9月西安交通大学附属红会医院收治的76例中青年股骨颈骨折患者临床资料, 其中男45例, 女31例;年龄23~63岁[(49.5±8.2)岁]。骨折Garden分型:Ⅱ型25例, Ⅲ型34例, Ⅳ型17例。骨折Pauwels分型:Ⅰ型16例, Ⅱ型31例, Ⅲ型29例。40例采用空心螺钉固定治疗(空心螺钉组), 36例采用FNS固定治疗(FNS组)。比较两组手术时间、透视次数、住院时间、下地负重时间、骨愈合时间。术后6, 12个月采用髋关节Harris评分标准评估两组患者髋关节功能。观察术后并发症发生情况。结果患者均获随访12~20个月[(15.2±1.7)个月]。FNS组透视次数为(13.4±1.9)次, 少于空心螺钉组的(18.2±2.6)次;FNS组下地负重时间、骨愈合时间为(11.1±1.9)周、(13.8±1.6)周, 短于空心螺钉组的(15.7±1.6)周、(14.6±1.6)周(P均<0.05或0.01)。两组手术时间、住院时间、术后6, 12个月髋关节Harris评分差异无统计学意义(P均>0.05)。两组患者术后均未出现感染。空心螺钉组4例股骨颈短缩, 3例深静脉血栓形成, 2例内固定松动, 2例退钉, 1例骨折不愈合, 并发症发生率为30%(12/40);FNS组2例股骨颈短缩, 1例深静脉血栓形成, 1例内固定松动, 并发症发生率为11%(4/36)(P<0.05)。结论对于中青年股骨颈骨折, FNS和空心螺钉治疗均可获得满意疗效, 但FNS具有术中透视次数少、下地负重时间早、骨愈合时间短及并发症发生率低等优势。
