血清DKK1、Chemerin、N-MID与乳腺癌骨质疏松骨密度的相关性

目的 研究血清分泌蛋白(DKK)1、视黄酸受体应答因子2(Chemerin)及骨钙素N端中分子片段(N-MID)与乳腺癌骨质疏松(OP)骨密度(BMD)的相关性。方法 选择乳腺癌患者140例纳入试验组,选择同期医院体检的健康女性140例纳入对照组,按照是否发生OP将试验组患者分为OP组、无OP组。观察收集各组年龄、体质量指数(BMI)及绝经情况;检测各组血清DKK1、Chemerin、N-MID水平及BMD。以Pearson相关分析基线资料及血清DKK1、Chemerin、N-MID水平与BMD的相关性;以多因素Logistic回归模型分析导致乳腺癌OP患者BMD降低的独立危险因素。结果 试验组血清DKK1、Chemerin、N-MID水平高于对照组,股骨干BMD、大粗隆BMD、1~4腰椎BMD均低于对照组,差异显著(P<0.05)。OP组TNM高分期比例、病灶高分级比例、绝经比例及血清DKK1、Chemerin、N-MID水平高于无OP组,股骨干BMD、大粗隆BMD、1~4腰椎BMD均低于无OP组,差异显著(P<0.05)。年龄、TNM分期、病灶分级、绝经及血清DKK1、Chemerin、N-MID与股骨干BMD、大粗隆BMD及1~4腰椎BMD均呈负相关(P<0.05)。年龄≥60岁、TNM分期Ⅲ期及以上、病灶分级G3级、绝经、DKK1、Chemerin、N-MID为导致乳腺癌OP患者BMD降低的独立危险因素。结论 血清DKK1、Chemerin、N-MID水平或可作为乳腺癌OP患者BMD降低的预测指标。
