The need and effectiveness of a national simulation training program in obstetric emergencies from a trainee view

摘要
Objective: Simulation training is a highly valuable and effective tool that has been increasingly used in the field of obstetrics to enhance knowledge and experience. We aimed to evaluate the immediate impact of a 1 day trainee initiated national emergency obstetric simulation course on clinical knowledge and self-assurance of trainees. Methods: This was a cross-sectional study conducted in the Ankara Bilkent City Hospital between December 2021 and March 2024. A total of eight courses were conducted with 200 trainees. Two online, anonymous surveys were created as pre- and post-tests of the course, consisting of the same 32 questions, filled in by the trainees before and after the course. Results: A total of 192 people participated in the study and 171 people (167 residents and 4 young specialists) were included. Participants completed a 41 question test that assessed four key areas; management of postpartum bleeding, operative vaginal delivery, breech delivery and shoulder dystocia, both before and after the training. The majority (93.6%) had never attended an emergency obstetrics simulation training. When the test results of the participants before and after the training were compared, a statistically significant improvement was found in all topics (P < 0.001). It was observed that the greatest improvement was in shoulder dystocia (45%), followed by breech birth management (40%). Participants with prior simulation experience showed lower improvement in some areas compared to those without prior experience (P < 0.001). Trainees also reported that "they feel more confident," and "they are more ready to manage these cases." Conclusion: Emergency obstetric simulation training remarkably improves knowledge and self-assurance among postgraduate obstetrics and gynecology residents. Continued implementation of national programs are recommended to ensure optimal preparedness and standardization in emergency obstetrics.
