Acute appendicitis remains a common surgical emergency necessitating appendectomy, a procedure with potential complications, including the rare occurrence of fistula formation. We present the case of a 35-year-old male who developed a fistula one-year post-appendectomy, characterized by feculant discharge. Imaging and endoscopic findings revealed a fistula extending from the cecum to the skin, prompting surgical intervention. Despite initial conservative measures, recurrence necessitated wedge resection. This case underscores the rarity and challenges in managing post-appendectomy fistulas, highlighting the lack of standardized protocols and emphasizing the importance of individualized approaches. Further documentation and dissemination of such cases are crucial for enhancing understanding and optimizing outcomes in similar scenarios.
