Next-generation diagnostics will significantly impact the level of treatment for refractive patients. Improvements in anterior segment imaging, using Scheimpflug-analysis and optical coherence tomography (OCT), are providing ophthalmologists with tools that allow them to evaluate pathologic corneas or corneas after refractive surgery as well as the dimensions of the anterior chamber. Besides classic PRK, further techniques of surface ablation are available (LASEK, EPI-LASIK). With modified preparation of the epithelial-flap (with or without repositioning) and modulation of woundhealing by the use of Mitomycin C, the occurrence of haze can be reduced. In postoperative comfort and visual recovery, LASIK or FEMTO-LASIK is still superior. For certain indications (e. g. thin corneas), surface ablation is still the method of choice. © Springer-Verlag 2007.
