High-density lipoprotein (HDL) reduces the renal injury and dysfunction in shock

Here we investigate the effects of reconstituted, human HDL (rHDL) on the circulatory failure and the renal injury and dysfunction associated with endotoxic shock or hemorrhagic shock in the anesthetized rat. Endotoxemia for 6 h resulted in a substantial increase in the serum levels of urea and creatinine as well as histological signs of renal injury. Administration of rHDL (80 mg/kg i.v.) 5 min prior to endotoxin largely attenuated the renal injury and dysfunction caused by endotoxin. Hemorrhage (sufficient to lower mean arterial blood pressure to 50 mm Hg for 90 min) and subsequent resuscitation with shed blood resulted (within 4 h) in injury and dysfunction of the kidney. Administration just prior to resuscitation (that is after 85 min of hemorrhage) of rHDL (80 mg/kg i.v.) abolished the renal injury/dysfunction. Thus, rHDL attenuates the renal injury and dysfunction with shock.
