Evaluation of consciousness by the Glasgow Coma Scale in aneurysmal subarachnoid hemorrhage. Appraisal using logistic regression analysis

The application of the Glasgow Coma Scale for evaluation of severity in subarachnoid hemorrhage was reviewed using logistic regression model 304 patients who underwent early surgery. The World Federation of Neurosurgical Societies grading failed to improve the previous Hunt and Hess grading because outcome was not significantly different in adjacent grades except for the border between Grade I and II. Three responses in the Glasgow Coma Scale appeared unequal, and logistic regression analysis revealed that the verbal response presented powerful influence to outcome. The use of eye-motor-verbal profile is recommended to achieve a strong correlation between the outcome and grades in subarachnoid hemorrhage, with mild consciousness disturbance in particular.
