Our method of reconstruction of pylorus-preserving pancreatoduodenectomy

To prevent severe gastric retention of foods after pylorus-preserving pancreatoduodenectomy (PpPD), we have been using the following surgical techniques. Traverso's reconstruction has advantages of creating an independent payhway of foods and an anastomosis with the jejunum exhibiting no congestion. We perform Braun's anastomosis to reduce entero-gastric reflux. Furthermore, the stomach is surgically fixed to maintain the position and shape of the organ. Over the last twe years,21 patients were treated with the above method and 6 patients with other reconstruction methods. The number of malignant cases were greater and the right gastric artery was more frequently sacrificed in the group of Traverso's reconstruction. however, the amount of gastric drainage was small, the nasogastric tube was removed 6.2 days after surgery, and the resumption of oral intake was achieved as early as 8.7 postoperative days in the group of Traverse's reconstruction.
