Use of a hearing loss grading system and an owner-based hearing questionnaire to assess hearing loss in pet dogs with chronic otitis externa or otitis media

BackgroundHearing loss is important when assessing the suitability of dogs with otitis externa/media for medical or surgical therapy. Hypothesis/objectivesTo assess an owner-completed questionnaire as an indicator of hearing loss and a canine hearing loss scoring system in chronic canine otitis. AnimalsOne hundred hospital population dogs referred for chronic otitis investigation. MethodsOwners completed a questionnaire to assess their dog's response to common household noises. The presence of otitis externa or media was determined and brainstem auditory-evoked response measurements were performed on each dog. The minimal hearing threshold (MHT) in decibels normal hearing level (dB NHL) was recorded and categorized according to the human World Health Organization grading system into five grades from 0 to 4 with cut-off values of 25dB NHL, 26-40dB NHL, 41-60dB NHL, 60-80dB NHL and 81dB NHL. ResultsThe questionnaire correctly determined normal hearing in grade 0 cases, but did not reliably detect unilateral or grade 1 bilateral hearing loss. For dogs with bilateral hearing loss grade 2, questionnaire sensitivity was 83% [24 of 29, 95% confidence interval, (CI) 64-94%] and specificity was 94% (67 of 71, 95% CI 86-98%). Higher grades of hearing loss were significantly associated with the presence of otitis media (P<0.01). Conclusions and clinical importanceThe questionnaire may be a useful in-practice screening tool in chronic canine otitis for moderate to severe bilateral hearing deficits (MHT 41dB NHL). The hearing loss grading system may help clinicians make therapeutic decisions. Chronic otitis media may be associated with higher grades of hearing loss. Resume ContexteLa prise en compte de la perte auditive est importante lorsqu'on evalue l'opportunite d'un traitement medical ou chirurgical des chiens atteints d'otite externe ou moyenne. Hypotheses/ObjectifsEvaluer un questionnaire complete par les proprietaires en tant qu'indicateur de perte auditive et un systeme de notation de perte auditive canine des otites chroniques canines. SujetsCent chiens issus d'une population hospitaliere, referes pour otite chronique. MethodesLes proprietaires ont complete un questionnaire pour evaluer la reponse de leur chien aux bruits communs de la maison. La presence d'une otite externe ou moyenne a ete determinee et les mesures de la reponse au potentiel evoque auditif ont ete realisees sur chaque chien. Le seuil d'audition minimal (MHT) en dB NHL (decibels normal hearing level) a ete enregistre et classe selon le systeme de notation de l'Organisation Mondiale de la Sante de l'homme en cinq grades de 0 a 4 avec des valeurs limites 25 dB NHL, 26-40 dB NHL, 41-60 dB NHL, 60-80 dB NHL et 81 dB NHL. ResultatsLe questionnaire a correctement determine une audition normale pour les cas a grade 0 mais n'a pas detecte de maniere fiable la perte d'audition unilaterale ou bilaterale de grade 1. Pour les chiens atteints de perte auditive a un grade 2, la sensibilite du questionnaire etait de 83% [24 sur 29, 95% d'intervalle de confiance, (CI) 64-94%] et la specificite etait de 94% (67 sur 71, 95% CI 86-98%). Le systeme de notation de perte auditive pourrait aider les cliniciens a faire un choix therapeutique. L'otite chronique moyenne pourrait etre associee avec des grades plus eleves de perte auditive.
