Thoracic Aneurysms aorta (AAA) are mostly asymptomatic of fortuitous discovery. Their etiologies differ from atheromathous abdominal aneurysm by the ascendancy of the genetic factors. A better knowledge of the physiopathological and genetic anomalies as well as the progress of techniques of medical imaging contributed to a better understanding of the various clinical forms. The most frequent complication of the AAA stays the aortic dissection/break, influenced by morphological and etiologic parameters. The aim of the treatment is to slow down the progress of the aneurysm size, it is based on the coverage of the cardiovascular risk factors and the prescription of a betablocker treatment. The renin-angiotensin inhibitor systems are assessed in this setting and could have a beneficial effect on the evolution arterial wall lesions. A regular follow-up by imaging is necessary. Fast increase in AAA diameter and most frequently complications encountered in the genetic forms leads to propose a prohylactic surgery prematurely.
