Between June 1996- August 2000, 439 anterolateral thigh flaps have been harvested based on musculocutaneous perforators in Chang Gung memorial Hospital. Complete success rate was 96.58%. Thirty-four flaps were re-explored and 19 were salvaged. In this study, some of the reasons of the flap failure, unique to anterolateral thigh perforator flap are identified and technical pearls in the harvest of anterolateral thigh perforator flap are discussed.
