摘要：

目的探讨柴胡加龙骨牡蛎汤联合氟哌噻吨美利曲辛片对卒中后抑郁(post-stroke depression, PSD)患者血清脂联素(adiponectin, APN)水平及抑郁状态的影响。方法将符合入选标准的150例PSD患者采用随机数字表法分为2组, 每组75例。在常规脑卒中治疗基础上, 对照组口服氟哌噻吨美利曲辛片, 观察组在对照组基础上加服柴胡加龙骨牡蛎汤。2组均治疗1个月。采用ELISA法检测APN浓度, 采用汉密尔顿抑郁量表(Hamilton Depression Rating Scale, HAMD)评估抑郁程度, 采用美国国立卫生研究院卒中量表(National Institutes of Health Stroke Scale, NIHSS)评估神经功能缺损程度, 评价临床疗效。结果观察组总有效率为81.3%(61/75)、对照组为58.7%(44/75), 2组比较差异有统计学意义(χ2=8.127, P=0.004)。治疗后, 观察组血清脂联素[(7.8±3.7)mg/L比(6.1±3.3)mg/L, t=2.970]水平高于对照组(P<0.05);HADM评分[(16.4±3.6)分比(19.6±4.9)分, t=4.558]、NIHSS评分[(5.0±1.6)分比(7.7±2.0)分, t=9.129]均低于对照组(P<0.01)。结论柴胡加龙骨牡蛎汤联合氟哌噻吨美利曲辛片可降低PSD患者的血清脂联素水平, 改善患者抑郁状态及神经功能缺损程度。
