APL患者血清miR-125b、NRF2水平与三氧化二砷疗效间关系及预测价值

摘要
目的 探讨急性早幼粒细胞白血病(APL)患者血清微小RNA(miR)-125b、核因子E2相关因子(NRF)2水平与三氧化二砷(ATO)疗效间关系及预测价值。方法 选取APL患者60例作为观察组,根据含ATO方案治疗1个疗程疗效分为高、低反应亚组。选取同期健康体检者60例作为对照组。比较观察组、对照组及高、低反应亚组血清miR-125b、NRF2水平。受试者工作特征(ROC)曲线分析血清miR-125b、NRF2水平对ATO疗效的预测价值。分析血清miR-125b、NRF2水平与APL患者ATO疗效的独立关系及交互作用。结果 观察组治疗1个疗程血清miR-125b、NRF2水平明显低于治疗前,观察组治疗前、治疗1个疗程血清miR-125b、NRF2水平明显高于对照组(P<0.05)。观察组治疗1个疗程显效35例(58.33%),纳入高反应亚组,其余25例(含4例死亡)纳入低反应亚组。治疗前、治疗1个疗程低反应亚组血清miR-125b、NRF2水平明显高于高反应亚组(P<0.05)。miR-125b、NRF2联合预测APL患者ATO疗效的AUC值高于单独预测的AUC值(P<0.05)。调整混杂因素后,miR-125b、NRF2高水平APL患者ATO低反应风险分别是miR-125b、NRF2低水平者2.669、2.415倍(P<0.05)。miR-125b与NRF2对APL患者ATO疗效存在正向交互作用。结论 APL患者miR-125b、NRF2是ATO低反应的危险因素,且两指标对患者疗效有交互作用,临床可通过检测miR-125b、NRF2判断患者治疗效果。
