RELATIONSHIP OF URIC ACID AND PROGNOSIS WITH CEREBROVASCULAR DISEASE

Cerebrovascular disease (CVD) is a clinical entity with focal neurological symptoms and signs, which represents the first cause of physical disability in adults, and the second global cause of death; so, its prevention is a priority. With the objective of determining the relationship of uric acid and its prognosis with cerebrovascular disease, a clinical, prognostic, analytical, longitudinal and prospective study was carried out. A sample was included which consisted of 200 patients of legal age, with cerebrovascular disease, and with clinical manifestation and imaging studies that were compatible with cerebrovascular disease. These patients were admitted to the Internal Medicine Service of the Central Hospital of Maracay, Aragua state, Venezuela, during the period January 2023 - June 2023. The results reported a predominance of males (53.50%), with the most frequent etiology of ischemic stroke, followed by hemorrhagic stroke (43%). Uric acid was found at 4.58 +/- 1.65 mg/dl and presence of systemic arterial hypertension in 99.50%. Hemorrhagic CVD was more frequent among patients with low levels of uric acid, while ischemic CVD was observed in patients with high levels of uric acid (p < 0.001), both extremes presented the highest associated mortality in the 2 CVD groups (p < 0.001). It is concluded that there is a direct, bimodal, significant relationship between uricemia and functional dependence by Rankin for both ischemic and hemorrhagic CVD with high and low uric acid concentrations, respectively, triggering poor short- and long-term prognosis. The poor prognosis increases with increasing age and stroke severity scale.
